How the out-of-pocket math works
A typical plan year moves through deductible spending, then coinsurance or copay cost sharing, until the member reaches the plan's in-network out-of-pocket maximum. Premiums remain separate from that ceiling.
Deductible phase = min(Allowed expenses, Deductible)
Coinsurance phase = your rate × remaining allowed expenses, capped by the remaining OOP max
Expected year cost = Annual premiums + your capped cost share

Frequently asked questions
Do premiums count toward the out-of-pocket maximum?
No. Premiums are separate from cost sharing, so the highest in-network covered-care exposure is the OOP maximum plus the premiums paid for the plan year.
Is the deductible included in the out-of-pocket maximum?
For ACA-regulated plans, covered in-network deductible spending is part of the annual cost-sharing structure and accumulates toward the applicable maximum.
What should I enter for expected expenses?
Use a reasonable estimate of covered, in-network allowed amounts rather than provider list prices or billed charges.
Does this model work for every health plan?
No. Copay-first designs, family accumulators, separate pharmacy rules, out-of-network benefits, excluded services, and other plan-specific mechanics can change the result.
